NURSE

Whenever a child will be late or absent, the parent is required to notify the school.  Students are not allowed to make the calls. Calls can be made to report the absence at any time, day or night, as the message machine is always in operation.  However, to ensure accurate attendance information, the school must be informed of your child's absence before 9:00 am on that day. 

To report the absence, call (516) 441-4115. Leave the following information:

Child's Name, Grade, Teacher's Name, Reason for Absence, and Expected Length of Absence or Expected Time of Arrival 

If the Health Office does not hear from you, an automatic call via ParentLink will be made to your home or place of work to verify the child's absence.  Numbers, where you can be reached, must be listed in the Parent Portal.

Upon returning to school, the child must provide a note from the parent explaining the reason for the absence.

Religious Absences
If a child will be absent from school for religious observance, New York State Law requires that the school receive a note to that effect BEFORE the absence takes place

PHYSICAL EXAMINATIONS

Physical Exam Form 2018-19

Physical exam forms are required for all students who are new to our district and grades Pre-K, K, 2, 4, 7, and 10. Camp physicals will satisfy this requirement. The forms are given out at the time of registration and in the spring (sent home with your child) and must be submitted within 30 days after entrance into the school or grade as mandated by New York State.

STATE MANDATED IMMUNIZATION

Immunization Changes for the School Year

Under section 2164 of the New York State Public Health Law, all children attending a school, day care center, or any preschool program must be immunized against Diptheria, Polio, Measles, Mumps, Rubella, Haemophilus Influenza b, and Hepatitis b. Children who attend a preschool or day care program must show evidence of Lead Screening. 

 Minimum requirements include: 

• DTP, DTaP, or Td: 3 doses 
• IPV, OPV OR EIPV: 3 doses 
• MMR (Measles, Mumps, Rubella): 2 doses
• ADMINISTERED AFTER 12 MONTHS OF AGE, or show certification of disease 
• HBV (Hepatitis b): 3 doses 
• Varicella 1 dose children born on or after 1/1/94 
• Pneumococcal Vaccine (PPV) 
• Prevnar 

A Health Services Registration Card should be on file in the Health Office. This form shows all required immunizations and a Health History of your child.

Students will not be permitted to attend gym or outdoor recess until a current physical examination is on file in the Nurse's Office.

ALL DATES (month, day, year) ARE REQUIRED. Your child may not attend school without this information.

Throughout the school year, VISION, HEARING and SCOLIOSIS screenings are done while your child attends school. It is important to remember, the school is not a diagnostic facility. Should there be concerns for further evaluations, a letter will be sent to you to seek an evaluation by your child's private health care provider.
