Health Examinations
It is required to submit a current physical examination for your child with current immunizations and medication each year.
Examinations are good for ONE year from the date of the exam. Students will not be permitted to attend Physical Education or recess until a current health report has been received.
In addition to the required physical examination forms, it is strongly recommended that children undergo dental check-ups as well as frequent eye examinations.
